Our Home Assistance Program is currently seeking a full time (35 hours per week) Case Manager.
The ideal candidate will evaluate potential participants as well as assist in outreach to increase community awareness of program services. Duties include evaluating potential participants, provide case management services and develop individual multi-disciplinary care plans and serve as a team member in the placement process.
This full-time position (35 hours per week) also offers a full benefit package, including a generous time off package. The work schedule is Monday through Friday, first shift hours.
B.S. in Human Service related field is required and previous experience in working with disabled/elderly adults in a community setting is preferred.
