We are seeking a part time (15 hours per week; Monday through Friday) Functional Skills Trainer. The Functional Skills Trainer will be responsible for providing PCA consumers with the information and training needed to successfully manage the PCA program and maximize the ability to self-direct care.
Responsibilities will include intake and orientation services to new referrals in accordance with DMA regulations including initial telephone response, eligibility screening and coordination of evaluation activities with the RN; conduct initial assessment of consumer's ability to manage PCA services independently; provide training to consumers and surrogates, if applicable, in the function and scheduling of PCA services to meet consumers' needs.
The schedule is Monday through Friday, first shift hours.
A High School diploma is required and experience with PCA or homecare.
